Meta-integration of qualitative studies of psychological experiences of children with a breast cancer mother

Objective To systematically evaluate qualitative studies on the experiences related to the integration of children with a breast cancer mother and to provide a basis for further family rehabilitation interventions. Methods We searched databases including the PubMed,Web of Science,the Cochrane Library,Embase,CINAHL,Joanna Briggs Institute(JBI),CNKI,Wanfang database,VIP Database and CBM to collect qualitative research on the experience or feelings of children after their mothers suffered from breast cancer. The retrieval period was from the establishment of the database to October 2022. The quality of the included studies was evaluated based on Joanna Briggs Institute Critical Appraisal Tool for qualitative research(2020 edition). The results were integrated by the method of aggregative integration. Results A total of 10 papers were included,and 49 outcomes were extracted into 10 categories and synthesised into 4 integrated outcomes:being aware of the changes and new needs brought about by breast cancer;experiencing negative emotions and genetic worries;trying to cope with life after mother’s breast cancer;discovering the positivity that comes from breast cancer. Conclusion Healthcare professionals should pay attention to the psychological experience of children with a breast cancer mother and provide appropriate psychological support and health education to promote their positive adjustment,thereby facilitating the recovery of breast cancer families.

Summary of best evidence for prevention and control of multidrug-resistant organism infection in adult ICU

Objective To search and summarize the best evidence for the prevention and control of multidrug-resistant organism infection in adult ICUs,and to provide evidence-based basis for clinical standard control. Methods We systematically searched relevant guideline websites and professional association websites,UpToDate,BMJ best practice,National Institute for Health and Care Excellence,National Guidelines Clearinghouse,Registered Nurses’ Association of Ontario,Yimaitong,the Cochrane Library,Embase,PubMed,Web of Science,CNKI,Wanfang,VIP and SinoMed to collect all the clinical guidelines,system reviews,expert consensuses,evidence summaries and original research related to prevention and control of multidrug-resistant organism infection in adult intensive care units. The search period was from the establishment of the database to April 30,2023. 2 researchers independently conducted the literature quality evaluation and data extraction. Results A total of 19 articles were included,including 1 guideline,8 expert consensuses,1 expert opinion,2 systematic evaluations,and 7 original studies. Ultimately,10 evidence themes and 41 pieces of best evidence were formed,including risk factors,hand hygiene,decolonization,active monitoring,equipment related strategies,contact protection,patient isolation,environmental cleaning,personnel training management supervision,and multidisciplinary collaborative management. Conclusion This study summarizes 41 pieces of best evidence for the prevention and control of multidrug-resistant organism infections in adult ICUs,which can provide resource preparation for clinical transformation and clinical decision-making basis for nursing staff to conduct scientific and effective nursing practices. Specialized nursing staff should reasonably select the best evidence for localized application,continuously update the best evidence,standardize nursing practices,and improve nursing quality.

Objective Systematic integration of qualitative research on couple intimacy in breast cancer patients was conducted to provide a foundation for psychosocial interventions involving breast cancer patients and their spouses. Methods For the qualitative studies of couple intimacy in breast cancer patients,a computer search was carried out on PubMed,Web of Science,CINHAL,Embase,the Cochrane Library,Joanna Briggs Institute(JBI),CNKI,Wanfang Database,VIP Database,and CBM. The retrieval time was from the establishment of the databases to October 2022. The quality of the literature was evaluated by the 2017 JBI Evidence-Based Health Care Center Quality Evaluation Criteria,and the results were integrated using the convergent integration method. Results 13 studies were included,and 55 results were extracted into 14 categories. 3 outcomes were combined,including conflict and harmed intimacy,modified and adjusted intimacy,and balanced and sublimated intimacy. Conclusion Patients who have breast cancer may experience numerous changes in their intimate relationships. Medical professionals should promptly understand the emotions of breast cancer patients and their spouses. Then,they should make targeted,effective interventions and support to encourage the growth of stable intimacy,which will help the couple cope with cancer better.

Objective To systematically review the real life experiences of nontraumatic lower extremity amputa-tions(NLEA) patients. Methods We searched databases,including PubMed,the Cochrane Library,Embase,Web of Science,CINAHL,PsycINFO,CNKI,Wan Fang data,VIP,and CBM from inception to August 2022 to collect qualitative studies on the real life experience of NLEA patients. The quality of included research was evaluated by the JBI Critical Appraisal Tool for qualitative studies from Australia. The results were summarized and integrated by the pooled integration methods. Results A total of 11 studies were included. 43 complete findings were grouped according to their similarities to form 3 integrated categories. The integration results were finally obtained. Integration results 1:facing multiple pressures and challenges,breaking the normalcy of life;integration results 2:there are multiple unmet needs;integration results 3:adopting different strategies to deal with amputation. Conclusion Nurses should pay attention to and understand the patient’s positive psychological state,and cooperate with available support to reduce the unmet needs of patients. It will be easier to reduce the transitional stress of the family,to help patients return to society better and improve their quality of life.

Objective To evaluate and summarize the best evidence for ultrasound-guided nasointestinal tube placement in adult critically ill patients,and to provide references for clinical practice. Methods The relevant evidence of ultrasound-guided nasointestinal feeding tube placement in adult critically ill patients was systematically searched from domestic and foreign databases and websites,including guidelines,expert consensuses,clinical decision-making,evidence summaries and systematic reviews,and the search time limit is from the establishment of the databases to June,2022. There were 2 researchers who independently assessed the quality of the literature and identified the level of evidence. Results A total of 22 articles were included,including 5 guidelines,6 expert consensuses,1 group standard,2 clinical decision-making,3 evidence summaries and 5 systematic reviews. 26 pieces of evidence were summarized from 5 themes:clinical situation,assessment,preparation,key points of catheterization,and nursing essentials. Conclusion The best evidence of ultrasound-guided nasointestinal tube placement in adult critically ill patients was summarized to provide evidence-based references for the implementation of bedside ultrasound-guided nasointestinal feeding tube placement and ensure patient safety.

Research progress on loneliness in patients undergoing hematopoietic stem cell transplantation during protective isolation

Loneliness is common in patients undergoing hematopoietic stem cell transplantation (HSCT) during protective isolation,which could affect their health outcomes. This article reviews the latest studies on loneliness,including concept,theory,influencing factors,assessment tools,and interventions. Currently,there are few studies on loneliness in HSCT patients in protective isolation in China and other countries. We propose to conduct some research to investigate the current status of loneliness,the influencing factors,and the relationship with patients’ mental health problems in the future,which could provide a theoretical basis for developing interventions to reduce the loneliness of the HSCT patients in protective isolation.

Expert consensus on packaging techniques of reusable medical instruments

Objective To standardize the packaging techniques of reusable medical instruments in central sterile supply departments. Methods From October 2021 to August 2022,the Professional Committee of the Central Sterile Supply Department of Chinese Nursing Association addresses the key problems of reusable medical instruments packaging. On the basis of clinical practice,literature analysis and the operation experience of the national central sterile supply department experts,we made the initial version of Expert Consensus on Packing Techniques of Reusable Medical Instruments(hereinafter referred to as the Consensus). Experts were invited to complete 2 rounds of Delphi expert consultations and 3 rounds of expert meetings. Each item was adjusted,revised and improved,and the final version of the Consensus was formulated. Results The positive coefficients of experts in 2 rounds of Delphi expert consultations were all 100%,and the degrees of expert authority were 0.957 and 0.973,respectively. The average importance value of each item was higher than 4.00;the variation coefficient of each item was less than 0.25. The Kendall’W were 0.181 and 0.338,respectively(all P<0.001). Finally,an agreement was reached on the terms and definitions,basic principles,personnel requirements,environmental requirements,packaging equipment requirements,packaging materials and sterilization monitoring material requirements,packaging auxiliary tools requirements,pre-packaging evaluation,packaging operation and packaging quality confirmation of the reusable medical devices packaging techniques. Conclusion The Consensus covers all aspects of the reusable medical instruments packaging techniques,which is highly practical and scientific,and it also helps to guide the sterile supply professionals to standardize the implementation of packaging techniques,improve the quality of aseptic supplies,and ensure the quality of medical treatment and the life safety of patients.

The effect of early oral feeding in patients with acute non-variceal upper gastrointestinal bleeding:a systematic review and Meta-analysis

Objective The purpose of this meta-analysis was to systematically evaluate the safety and effectiveness of early oral feeding(received oral diet from day 1 after hemostasis) on patients with acute non-variceal upper gastrointestinal bleeding(ANVUGIB),and provide evidence references for clinical practice. Methods 8 electronic databases including SinoMed,China National Knowledge Infrastructure(CNKI),Wanfang data,VIP data,PubMed,the Cochrane Library,CINAHL,and Embase were searched for English and Chinese articles published on or before September,2022. There were 2 reviewers who retrieved articles,evaluated quality and extracted data independently. Review manager(RevMan) version 5.4 and Stata version 13.0 were then used to perform Meta-analysis. Results 4 RCTs and 1 cohort study with 803 cases of ANUGUIB were enrolled. Early oral feeding did not increase the rate of re-bleeding[RR=0.87,95%CI(0.47,1.64),P=0.670] or mortality[RR=0.46,95%CI(0.19,1.08),P=0.080];while the length of hospital stay was shortened[MD=-2.40,95%CI(-2.43,-0.12),P=0.030]. However,the hospitalization costs[SMD=-1.50,95%CI(-3.03,0.03),P=0.050] in the early oral feeding group were not signifi-cantly different from those in the conventional feeding group. Conclusion The existing evidence shows that early oral feeding in patients with ANVUGIB does not increase the risk of poor prognosis and mortality,but also it can shorten the length of hospital stay,with no significant effect on reducing the hospital costs of patients. The included studies are few;the sample size is small;the specific implementation of early oral feeding is different. Therefore,high-quality,large-sample,multicenter original studies are still needed to further verify the application effect of early oral feeding.

There is a large number of research on acute alcohol impairment by foreign researchers,but there are few relevant research reports in China. This article reviews the origin,development and the evaluation tools of acute alcohol impairment in the context of emergency treatment,and summarizing its advantages and disadvantages by comparing the content,evaluatio. Methods,applicable population,reliability and validity of each evaluation tool,in order to provide scientific references for the evaluation,treatment and nursing care of acute alcohol impairment for emergency departments in China.

Analysis on the status quo and influencing factors of clinical nurses’ disaster preparedness ability

Objective The current situation of disaster preparedness ability of clinical nurses in general hospitals was investigated and analyzed,and the possible factors affecting the disaster preparedness ability of nurses were discussed,so as to provide theoretical basis for carrying out targeted training to improve the disaster preparedness ability of nurses and guiding the dispatch of disaster rescue nurses. Methods From August to September 2022,with convenience sampling method and general information questionnaire and Readiness Estimate and Deploy Ability Index,clinical nurses from 2 to 3 tertiary general hospitals in each region were selected to conduct questionnaire survey. Results A total of 1 048 valid questionnaires were collected,and the total score of clinical nurses’ disaster preparedness ability was (118.71±29.17) scores,and the average score of each item was(3.21±0.79) scores. The scores of the items of identity were generally higher than those of the items of disaster rescue ability. Among the 6 dimensions of the scale,disaster care ability scored the lowest,with only(2.64±1.04) points. Nurses’ working years,educational background,batch treatment experience,confidence in completing diversified tasks and the unit’s normal first-aid training mechanism were the possible factors influencing clinical nurses’ disaster preparedness ability(P<0.05). Conclusion In this study,nurses’ disaster preparedness ability was at a medium level,and the scores of various dimensions and items of disaster preparedness ability were significantly different. Affected by various factors,nursing managers should attach importance to the training of nurses’ disaster preparedness ability,establish an evaluation,hierarchical training and assessment mechanism,and continuously improve nurses’ comprehensive ability to deal with emergencies.

Objective To search,evaluate,and synthesize the relevant evidence for the management of insulin intravenous infusion in ICU patients and summarize the best evidence. Methods We searched Guidelines International Network(GIN),National Institute for Health and Care Excellence(NICE),National Guideline Clearinghouse(NGC),Registered Nurse Association of Ontario(RNAO),Scottish Intercollegiate Guidelines Network(SIGN),BMJ Best Practice,JBI,Cochrane Library,PubMed,Embase,CINAHL,UpToDate,SinoMed,CNKI,Wangfang Database and clinical guidelines APP to collect clinical decisions,guidelines,evidence summaries,expert consensuses related to insulin intravenous infusion management of adult patients in ICU. Through literature quality evaluation,the evidence was extracted from the literature that met the quality standards. The search period is from the inception to August 31,2022. Literature screening and quality evaluation were conducted independently by 2 researchers,and the evidence was extracted and summarized according to the subject. Results A total of 8 articles were selected,including 2 guidelines,1 systematic review,4 evidence summaries and 1 expert consensus. Finally,19 pieces of best evidence were formed in 4 aspects,including blood glucose control target,blood glucose monitoring,insulin infusion and hypoglycemia disposal. Conclusion The study summarizes the best evidence for blood glucose monitoring,insulin infusion,and hypoglycemia management during intravenous insulin infusion in critically ill patients,which provides a basis for clinical nursing practice. Nursing staff can reasonably select the best evidence for localization application according to clinical situation to promote the management of ICU patients with intravenous insulin infusion.

Longitudinal study of kinesiophobia trajectory in patients with acute myocardial infarction after percuta-neous coronary intervention

Objective To explore the status and development trend of kinesiophobia after percutaneous coronary intervention(PCI) in patients with acute myocardial infarction(AMI),and to provide evidence for the best intervention time point of kinesiophobia. Methods From November 2020 to June 2021,a total of 105 patients with AMI after PCI admitted to the Department of Cardiology of tertiary A general hospitals in Hangzhou were selected by convenience sampling method. The general data questionnaire,tampa scale of kinesiophobia-heart and cardiac anxiety questionnaire were used to investigate and observe the degree of kinetophobia and psychogenic anxiety at 3 time points(within 1 week of hospitalization,2 weeks after discharge and 3 months after discharge). Results The kinesiophobia scores of AMI patients at 3 time points after PCI were(40.82±5.40),(41.90±4.60),(37.40±3.35),which were higher than the norm(32.20±7.40). The results of repeated measure ANOVA showed that different time,different age groups and cardiac anxiety significantly affected the score of kinesiophobia(P=0.022,P=0.048,P<0.001). After the control for covariates,different time had a significant interaction with age(P=0.013),T1 and T2 time had significant interaction with cardiac anxiety(P=0.001,P<0.001). Conclusion Due to the interaction of time, age and cardiac anxiety, the level of kinetophobia in AMI patients was higher after PCI and within 3 months after discharge. At T2 time point, the score of kinesiophobia and cardiac anxiety in the young and middle-age groups was significantly higher than those in the elderly group. The trajectory of kinesiophobia showed a slight increase in acute convalescence period,and a natural trend of gradual decrease in middle and post convalescence period. The kineophobia kinesiophobia of AMI patients after PCI lasts for a long time with a wide impact,so it is necessary to timely carry out interventions.

The application of a Doctor-Nurse-Patient Shared Decision-Making Assistance program in the prevention of ischemic stroke in patients with atrial fibrillation

Objective To study the application value of the doctor-nurse-patient shared decision-making assistance program in the prevention of stroke in patients with atrial fibrillation. Methods Through literature review and expert discussion,treatment decision aid tools were constructed. The convenience sampling was adopted,and 62 atrial fibrillation patients who were willing to participate in treatment decision-making in a tertiary hospital in Zhejiang Province from April to July 2021 were selected as the research subjects. The patients were divided into a test group and a control group,with 31 patients in each group. The test group adopts the doctor-nurse-patient joint shared decision-making assistance program,and the control group adopts the conventional plan. The actual participation in treatment decision-making of the 2 groups of patients,the satisfaction of patients with participating in treatment decision-making,the conflicts of patients’ decision-making,and the INR compliance rate of the 2 groups of patients were analyzed. Results There were statistically significant differences in the degree of patients’ actual participation in treatment decision-making between 2 groups(P<0.001).The test group was(42.26±6.38) points,and the control group was(26.48±7.31) points. There were statistically significant differences in patients’ satisfaction with participating in treatment decision-making between groups(P=0.04). The test group was(74.68±11.68) points,and the control group was(49.68±15.26) points. There were statistical differences in the decision-making conflicts of patients between groups(P<0.001). The test group was(24.81±6.72) points,and the control group was(45.39±8.47) points. There were statistically significant differences in the INR compliance rate of patients between 2 groups(P<0.001). The INR compliance rate of the test group was 54.17%,and the INR compliance rate of the control group was 14.29%. Conclusion The treatment decision-making assistance for patients with atrial fibrillation in the prevention of stroke have significant clinical significance on patients’ participation in treatment decision-making,treatment decision-making satisfaction,decision-making conflict and INR compliance rate.

Objective To retrieve the relevant evidence for the prevention and management of peristomal moisture-associated skin damage(PMASD) in adults,and to summarize the best evidence to provide a reference for formulating a scientific and reasonable skin care plan around the stoma. Methods According to the top-down principle of the evidence-based resource 6S model,a systematic search of domestic and foreign databases and websites was conducted on the prevention and management of PMASD in adults,including clinical practice guidelines,best clinical practice information manuals,evidence summaries,expert consensuses,systematic reviews,and standards. The search time limit was from the establishment of the databases to May 2021. 2 trained researchers in systematic evidence-based knowledge evaluated the quality of the literature,combined with professional judgment,selected the literature that met the inclusion and exclusion criteria,and extracted the evidence. Results A total of 14 articles were included,including 1 clinical decision support article,3 guidelines,1 standard,3 evidence summaries,3 systematic reviews,and 3 expert consensuses. The best evidence includes 20 recommendations in 6 aspects,namely positioning,evaluation,cleaning,selection and use of ostomy care products,skin care around the ostomy,and health education and the follow-up. Conclusion This study summarizes the best evidence for the prevention and management of PMASD in adults,and provides evidence-based references for clinical medical staff to carry out the application of targeted evidence.

Objective To assess the incidence and risk factors of postoperative nausea and vomiting(PONV) in liver cancer patients,and to provide references for clinical nursing intervention. Methods We used consecutive sampling method to select liver cancer patients who underwent liver resection at Zhongshan Hospital of Fudan University from October 2020 to April 2021. A total of 352 patients were prospectively studied. PONV was assessed after 2 hours,6 hours and 24 hours after surgery,respectively. Demographic data,disease information,and surgery information were collected to analyze the risk factors. Results The overall incidence of PONV in liver cancer was 48.30%(170 cases),of which 58.82%(100 cases) developed within postoperative 2 hours,20.00%(34 cases) developed between 3-6 hours after surgery,and 21.18%(36 cases) developed between 7-24 hours after surgery. Logistic regression identified the female,length of surgery,PONV history or motion sickness history,time of portal vein occlusion≥15 min and age<60 years old as risk factors of PONV. Conclusion The incidence of PONV within 24 hours in liver cancer was high.Nurses should pay much attention to those who were female at a younger age,with longer surgery,PONV or motion sickness history,longer time of portal vein occlusion,and take preventive measures to avoid or reduce PONV.

Intervention effects of rehabilitation training of mode of human occupation on self-efficacy and stigma of schizophrenia patients

Objective To explore the intervention effects of rehabilitation training of mode of human occupation (MOHO) on self-efficacy and stigma of convalescent schizophrenic patients. Methods 103 inpatients with schizophrenia were randomly divided into a research group(53 cases) and a control group(50 cases). The study group received a rehabilitation training of MOHO on the basis of anti-psychotic treatment and routine nursing, 5 times a week for 8 weeks. The control group received a routine rehabilitation training on the basis of anti-psychotic treatment and routine nursing. Before and after the intervention,the general self-efficacy scale(GSES) and the stigma scale for patients with mental illness(SSMI-C) were used to evaluate the effect of the intervention. Results The results of intra group comparison showed that GSES was(15.46±2.74) before intervention and(17.92 ±2.42) after intervention. Before the intervention,the total score of stigma was(52.12±7.53),discrimination was (21.36±6.83),disease concealment was (17.28±4.45),positive effect was(13.48±3.81);after the intervention,the total score of stigma was (41.66±4.95),discrimination was(16.84±4.02),disease concealment was (14.18±2.75),and positive effect was(10.64±2.38). The difference was statistically significant(P<0.001). The comparison results within the control group showed that the post test score of self-efficacy was higher than the pretest score,and there was no significant difference in GSES score,total score of stigma,discrimination,disease concealment and positive effect factor score(P>0.05). After the intervention,the GSES score of the research group was higher than that of the control group,and the total score and each dimension score of stigma were lower than those of the control group(P<0.001). Conclusion The rehabilitation training of MOHO can effectively reduce the level of stigma and improve the sense of self-efficacy.

The status quo and influencing factors of post competency among nurses in central sterile service departments

Objective To investigate the level of post competency among nurses in central sterile service departments(CSSD) and to analyze its influencing factors,so as to provide references for improving the quality construction for nursing teams in CSSD. Methods A total of 3 312 nurses in CSSD of hospitals in 31 provinces and autonomous regions were selected from October 2021 by the convenient sampling method. The nurses were investigated by the demographic information questionnaire and Post Competency Assessment Scale for nurses in CSSD. Multiple linear regression analysis was used to study the influencing factors of nurses’ post competency. Results The total score of post competency of nurses in CSSD was(4.32±0.55). Multiple linear regression analysis showed that gender,years of working in CSSD,hospital grade,number of operations per month,centralized management,energy level management and post performance management were the influencing factors of post competency of nurses in CSSD. Conclusion CSSD nurses generally mark high scores on the post competency assessment. Male CSSD nurses,longer working years,higher tier of hospitals,and monthly operations would tend to have higher post competency of nurses. For hospitals executing centralized sterile supply management,competence level management and post performance management,their nurses tend to score higher on post competency assessment as well. Managers should pay attention to the evaluation and intervention of post competency of nurses in CSSD,and formulate targeted post competency training plans based on related factors to improve the post competency of nurses in CSSD and promote the sustainable development of this special profession.

Objective To explore the risk factors of enteral nutrition-associated diarrhea(ENAD),and to construct a risk prediction model based on random forest. Methods This study adopted a retrospective data collection method. A total of 537 ICU patients who received enteral nutrition in a tertiary hospital in Tianjin from August 2017 to May 2021 were enrolled. Based on univariate analysis and multivariate logistic regression analysis and VIF,the variables of the risk prediction model for enteral nutrition-associated diarrhea were determined. All data are randomly divided into a training set and a validation set by 6:4. Based on the training set data,the random forest model of ENAD is established,and the importance of ENAD risk factors was ranked by variable importance score using a random forest model. Confusion matrix analysis,accuracy,sensitivity,specificity,and area under the ROC curve were used to evaluate the predictive effect of the model on the validation dataset. Results The incidence of ENAD in this study was 38.73%. After univariate analysis,multivariate Logistic regression analysis and VIF,9 variables were determined to be included in the prediction model. The top 5 variable importance scores of the random forest prediction model include days of taking potassium preparations,APACHE Ⅱ score,daily doses of enteral nutrition,fasting days before enteral nutrition and hypoproteinemia. The accuracy of the random forest model on the validation set is 76.27%;the sensitivity is 70.73%;the specificity is 79.70%;the area under the ROC curve is 0.810(95%CI:0.638~0.827). Conclusion The predictive model has good predictive performance,which provides a reference for the precise prevention of clinical nurses with certain practical significance.

Objective To explore the posttraumatic growth experience of grandparents of children with developmental disabilities under 6 years old,so as to provide a basis for formulating intervention programs to promote their psychological growth. Methods The purposive sampling method was adopted to select 12 grandparents of children with developmental disabilities who received the rehabilitation treatment in a tertiary A hospital in Chongqing from October 2021 to January 2022 as research subjects. Data were collected by semi-structured interviews and face-to-face nonverbal behavior observation method,and data were sorted out with the help of GooSeeker software. The data were analyzed by Colaizzi 7-step analysis method. Results 5 themes were extracted.①The positive psychological derivations:perceived progress of children,looking forward to a better future,positive coping styles.②New life perception:accepting reality calmly,shifting focus of life,valuing health more. ③The enhancement of personal strength:the enhancement of resilience,the enhancement of self-efficacy,optimism and prospect. ④The enhancement of support system:the enhancement of emotional support,the enhancement of social support.⑤Interpersonal harmony:altruistic behavior,improvements in relationships with others. Conclusion Grandparents of children with developmental disabilities under 6 years old have various experiences of posttraumatic growth. Medical staff can help them get more social support through group psychological counseling,setting up psychological support groups and strengthening disease publicity,so as to help them effectively cope with stress and promote posttraumatic growth.

Objective To construct a platform of post-hospital health management for mothers and infants based on “Internet +”,and to provide corresponding evidence. Methods The post-hospital health management for mothers and infants based on “Internet+” included 2 parts:①Patient APP included service items page and personal information page,supporting personal information registration,previous health file query,team selection and appointment,service evaluation,etc;②Medical APP included personal work station,appointment service application display and consultation communication interface,supporting medical staff registration,viewing appointment services,and consultation. Data from January 25 to August 31,2021 were collected,including the use of platform,online medical team-patient interaction,and maternal satisfaction with the platform. Results The platform has served 990 mothers,and the repeated use rate was 38.28%(379/990). The consultation content covers many aspects of maternal and child care during the puerperium period,of which a total of 42 cases were solved by medical and nursing staff (4.24%);251(25.36%) problems were solved off regular working hours. Conclusion Post-hospital health management platform for mothers and infants based on “Internet +” is convenient,efficient and professional,and it works well. Solving maternal and infant problems in a timely,professional and efficient manner,it improves the participation of mothers and meets their diversified health needs.

Objectives To retrieve,evaluate and summarize the best evidence for preventing and management of intra-abdominal hypertension in critically ill patients to provide evidence for clinical nurses to prevent and monitor the intra-abdominal hypertension in critically ill patients. Methods We searched websites and databases including,but not limited to,UpToDate,BMJ Best Practice,National Institute for Health and Clinical Excellence,Guidelines International Network,Registered Nurses Association of Ontario,Scottish Intercollegiate Guidelines Network,World Health Organization,Yimaitong guide network,World Society of the Abdominal Compartment Syndrome ( WSACS ),Cochrane library,PubMed,Embase ( OVID ),CINAHL ( EBSCO ),CNKI,Wanfang for guidelines,recommended practices,evidence summaries,expert consensuses,and systematic reviews,randomized controlled trials,best practice information manuals regarding monitoring and management of intra-abdominal hypertension in critically ill patients were searched and retrieved. 2 researchers independently assessed methodological quality of included papers and extracted data. Results A total of 15 articles were included,including 5 guidelines,1 expert recommendation,2 expert consensuses,1 best recommendation,6 systematic reviews. Finally,21 pieces of best evidence were formed from 6 aspects,namely the assessment,monitoring management,treatment management,posture management,enteral nutrition management,volume resuscitation and fluid management. Conclusion The best evidence for prevention and management of intra-abdominal hypertension in critically ill patients summarized in this study is scientific and comprehensive,providing the evidence-based basis for medical staff to standardized measurement and management of intra-abdominal hypertension in critically ill patients in clinical practice.

Objective To systematically review and synthesize the experience of nurses participating in robotic surgery,so as to provide recommendations for clinical practice. Methods The computer retrieval was performed in PubMed,CINAHL(EBSCO),Embase,Web of Science,Scopus,ProQuest Dissertations and Theses,SinoMed,CNKI,Wanfang Data and VIP database from January 1,2000 to November 26,2021,to collect qualitative studies in the experience of nurses participating in robotic surgery. This qualitative systematic review was conducted under the Joanna Briggs Institute guideline. This paper was written according to the enhancing transparency in reporting the synthesis of qualitative research(ENTREQ). Results A total of 6 qualitative studies were included,and 39 findings were extracted,which were summarized into 8 categories and integrated into 3 synthesized findings. Findings included that nurses play a coordinating role in teamwork and gain a sense of achievement;nurses face multiple challenges and pressures in their work;nurses lack professional knowledge and need technical support and team training. Conclusion The communication and cooperation among members of the robotic surgical team should be strengthened;the management of operation time,operating room space,equipment failure and patient safety should be strengthened;the operation process should be standardized;the legal responsibilities of nurses should be clarified;professional team training and technical support should be provided for nurses.

Objective To retrieve,evaluate,integrate the evidence of catheter related skin injuries prevention practices in cancer patients with PICC from home and abroad,and then summarize the relevant best evidence to provide evidence-based practice for clinical standardized management of catheter related skin injuries. Methods Evidence on prevention of catheter related skin injuries in cancer patients with PICC was systemically retrieved in the websites and databases,namely the BMJ Best Practice,UpToDate,Guidelines International Network,National Guideline Clearinghouse,National Institute for Health and Care Excellence,Scottish Intercollegiate Guidelines Network,Registered Nurses Association of Ontario,Intravenous Nurses Society,National Comprehensive Cancer Network,Oncology Nursing Society,medlive,Cochrane Library,Joanna Briggs Institute,PubMed,Embase,CINAHL,Taiwan academic literature database,CNKI,WanFang,VIP,SinoMed,including clinical decision-making,guidelines,evidence summaries,best practice,recommended practice,systematic reviews,expert consensuses,and government documents. The literature retrieval period was from the database construction to September 12,2021. The guidelines were individually evaluated by 4 experts,and the remaining literature was independently evaluated by 2 researchers. The literature that met the criteria was extracted and graded. Finally,the expert group integrated the evidence and summarized the evidence topics. Results A total of 12 articles were involved,including 1 government document,3 guidelines,4 evidence summaries,3 expert consensuses,and 1 systematic review. Finally,7 evidence topics and 26 pieces of best evidence were formed,including assessment,skin cleaning and antisepsis,dressing selection,operation and securement techniques,skin protection at PICC insertion site,patient education,supervision and training. Conclusion The best evidence of catheter related skin injuries in cancer patients with PICC can provide resources preparation for clinical transformation and offer nurses clinical decision-making for scientific and effective nursing practice. Specialist nurses should reasonably choose the best evidence for localized application,and constantly update the best evidence,standardize nursing practice,and ensure the safety of patients.

Objective To translate the short version of Glaucoma Medication Self-Efficacy Scale(GMSES) into Chinese,and to evaluate its psychometric properties in Chinese glaucoma patients. Methods After obtaining the consent of the author of the original scale,researchers conducted the translation and cross-cultural adaptation of the short version of GMSES in accordance with Brislin translation model,and the expert consultation and pilot test were also completed before full psychometric testing. A total of 244 glaucoma patients from 3 hospitals in Zhejiang province and Henan Province who received eye drops were recruited by convenience sampling. The reliability and validity of the Chinese version of the short GMSES(C-GMSES) were evaluated. Results For the overall C-GMSES,the Cronbach’s α coefficient was 0.837 and the test-retest reliability was 0.938. The Cronbach’s α coefficients of 3 dimensions were 0.741,0.933 and 0.817,respectively. The scale-level average content validity index(S-CVI/Ave) was 0.990 and the item-level content validity index(I-CVI) ranged from 0.833 to 1.000. A total of 3 common factors were extracted from exploratory factor analysis;the cumulative variance contribution rate reached 63.642%. Conclusion The C-GMSES has been proved to be reliable and valid,and it can be used to evaluate the medication self-efficacy of patients with glaucoma in China.

Objective To test the reliability and validity of Post Competency Assessment Scale for nurses in central sterile service departments(CSSD). Methods According to the Iceberg Competency Model,an evaluation index framework for the competency of nurses including 4 first-level indexes and 58 second-level indexes was formed through Delphi consultation in previous research. The members of the research group further transformed the indexes to a 60-item post competency assessment scale for nurses in CSSD. 6 experts assessed the content validity of the scale;from May to June 2021,34 nurses in CSSD from 3 hospitals in Beijing were selected for preliminary survey;from September to October 2021,630 nurses from 2 provinces were selected for formal investigation,so as to test the reliability and validity of scale. Results The scale contained 4 factors,including theoretical knowledge,operational skills,comprehensive ability and personal characteristics,with a total of 60 items. The content validity of the total scale was 0.970,and the scale-level content validity index/average were 0.830~1.000;4 factors were identified by exploratory factor analysis,accounting for 71.51% of the total variance. The overall Cronbach’s α coefficient was 0.989. The overall split‐half reliability was 0.951. The test-retest reliability was 0.840. Conclusion The Post Competency Assessment Scale for nurses in CSSD has good reliability and validity. It can be used as a tool to assess the post competency for nurses in CSSD,and a reference for hospitals to make training plans and annual assessment.

Objective To design a modular embedded storage device and explore its effect. Methods Quasi-experimental design was used in this study. Nurses from a cardiology department of our hospital used the modular embedded storage device to prepare supplies for nursing operation items(in an experimental group),while nurses from a neurology department used general storage cabinets to prepare nursing products(in a control group). Researchers tracked time spent on preparing 5 high-frequency nursing operation items in the 2 groups for 3 months. The preparation time of nursing supplies,the quantity and cost of out-of-date stock,and the satisfaction of nurses were compared between the 2 groups. Results Compared with it in the control group,the material preparation time in the experimental group was significantly shortened,and the difference was statistically significant(P<0.001). The total satisfaction score of the experimental group was higher than that of the control group,and there were statistical differences in time-saving,comfort,convenience,safety and accuracy of picking goods(P<0.05). There were 13 539 nursing materials expired in the control group in a year while only 3 invalid items in the experimental group,which were equivalent to 13,089 RMB and 45 RMB respectively. Conclusion The modular embedded storage device is useful to facilitate cost efficiency of the nursing supply chain,improve quality and safety of the working system,and improve nurses’ satisfaction. It is worth promoting.

After careful treatment and care,all 12 patients were discharged from the hospital with the ventricular assist system EVAHEART I.

Objective To develop the post competency index system for nurses in Central Sterile Service Department,so as to provide scientific bases for the homogenized,standardized training strategy and evaluation of the competence level of the nurses in Central Sterile Service Department. Methods According to the Iceberg Competency Model,the paper drew up an evaluation index framework for the competency of nurses and prepared the inquiry paper of expert letters based on methods as document analysis. 3 rounds of Delphi consultation questionnaires were sent to 20 experts from 11 provinces and cities in May 2020. They were experienced in areas of management of central sterile supply,and helped to define the post competency index system for nurses in Central Sterile Service and its weight assignment. Results The established post competency index system for nurses in Central Sterile Service consisted of 4 first-level indexes,including theoretical knowledge,operational skills,comprehensive ability and personal characteristics,58 second-level indexes after 3 rounds of Delphi consultations. The effective response rate was 100%. The three-round authority coefficient values were 0.88,0.88,and 0.89 respectively. The Kendall harmony coefficients of expert opinions of the 3 rounds of first and second-level indexes were 0.301,0.343,0.413 and 0.322,0.256,0.292,respectively,and P<0.001 after the consistency test. Conclusion The post competency model for nurses in Central Sterile Service Department constructed in the study is reliable,scientific and practical,which will pro-vide objective quantitative bases for post training and evaluation for nurses in Central Sterile Service Department.

Objective To investigate the status of adult patients with Cushing’s disease and its influencing factors,and to provide evidence for optimizing the quality of the patients.Methods The cross-sectional survey was carried out from November 2018 to June 2020 among adult patients with Cushing’s disease recruited by convenience sampling in a single center. Research instruments included the General Information Questionnaire,Chinese version of Cushing’s syndrome patients’ quality of life questionnaire,PHQ-9 depression screening scale,GAD-7 self-assessment scale,Karnofsky functional status rating scale. Data cleaning,statistical description,single factor analysis and multivariate regression analysis were completed with SPSS 23.0 software.Results The score of the quality of life in patients with Cushing’s disease was(46.49±20.58). The results of multivariate regression analysis showed that cortisol level,depression and anxiety were risk factors of low quality of life in patients with Cushing’s disease(P<0.05).Conclusion The quality of life of adult patients with Cushing’s disease is not high. Medical staff should pay more attention to patients with Cushing’s disease with high cortisol level,depression and anxiety,and try to prevent the occurrence of risk factors and implement targeted interventions and care as soon as possible once any risk factors are identified.

Objective To overview the systematic reviews of effectiveness of management of medication compliance to antiretroviral therapy(ART) in HIV/AIDS patients. Methods CNKI,CBM,JBI,The Cochrane Library,databases were searched to identify the relevant systematic reviews. 2 researchers screened systematic reviews and extracted the data independently. AMSTAR 2 scale and GRADE system were used to assess the quality of included systematic reviews. Results A total of 16 systematic reviews were included in this study. Results showed that the quality of 12 systematic reviews were rated as very low,and the quality of 4 systematic reviews were rated as low. The application of mobile technology can improve medication adherence of HIV/AIDS patients, and the evaluation methods of medication adherence are diverse. Conclusion The application of mobile technology can improve medication adherence of HIV/AIDS patients. However,existing studies are insufficient. The overall effectiveness of management of medication compliance to ART in HIV/AIDS patients needs to be further explored and validated by subsequent high-quality and large-sample studies.
